I contacted the publicity person at Hodder & Stoughton regarding both the two states of the proofs (and how scarce they seem to be) and the different dust jackets. Here is the response:

John - yes, there were two proof runs done - and yes, they were limited. I imagine that people are holding onto their copies - not least because it was a good book, and one they wish to keep!
We only had one finished jacket for the hardback - but maybe we proofed one in different coloured lettering before deciding to change it. I know that the lettering was white on the proof. Maybe that is the answer.
